					<description>Hi there, I am very interested in your Ultimate Form, and see that there is a 15 day trial, but what I can't find is the price of the software after the trial period.

I am using currently using CuteSiteBuilder and Total Choice Hosting, are you familiar with these.

How do I put multiple check box responses in the database?
Right now it's only picking up the last check.

i.e..   Your goals:
There there is a list of 5 they can choose from.

You are supposed to pick as many as apply to you, but when they hit &quot;submit&quot; 
only the last option they checked are listed in the database for that 
question.

Is there a way to put all the checked responses in the database?
