Hi there, I am very interested in your Ultimate Form, and see that there is a 15 day trial, but what I can’t find is the price of the software after the trial period.

I am using currently using CuteSiteBuilder and Total Choice Hosting, are you familiar with these.

Hi Jack,
I love the script - this is my second license. However I have run into a
problem -
How do I put multiple check box responses in the database?
Right now it’s only picking up the last check.

i.e.. Your goals:
There there is a list of 5 they can choose from.

You are supposed to pick as many as apply to you, but when they hit “submit”
only the last option they checked are listed in the database for that
question.

Is there a way to put all the checked responses in the database?
